Issue 4 - Damage LM5140 after Hot Plug DC Jack

Applicable EVM Revision: C, D

Issue Description: The U30 LM5140 is damaging after several iteration of Hot plugging DC power jack on to the EVM.

Solution: Replace damaged U30 LM5140QRWGTQ1, Add 17.4k 0402 resistor on top of C406.
